Most students will be able to use the 1040NR or 1040NR-EZ tax forms. These
tax forms are for U.S. Nonresident Aliens. The forms and their instructions

What if I have lost my passport?
You will need to contact your embassy or consulate in the United States
for instructions on how to renew or replace a lost passport.
What do I do if I have lost my I-94?
The I-94 is an Initial Nonimmigrant Arrival-Departure Document which is
necessary in order to apply for a Social Security Card in the US. If you
have lost your I-94, you must fill out form I-102 and send it along with
the $155 processing fee to the U.S Citizenship and Immigration office
which serves your area. Download form I-20 and instructions here:

Where do I get a Social Security Card?
In most cases, your employer will take you to the local Social Security
office to apply for your card. In other instances, you must complete an
Application for a Social Security Card (Form SS-5). To get an application:

• Visit
your local social security office
To get a Social Security card, you must provide original documents that
show your age, identity and citizenship or lawful non-citizen status
What do I do if I need help?
If you need help or have questions, feel free to call or email Apex Monday
through Friday 8:00am to 4:00pm Central Standard time. Please leave a
message after hours. In the case of emergencies, please call the phone
number provided in the handbook anytime day or night. The following situations
are considered emergencies: serious illness or injury, criminal arrest,
or victim of a crime.
What if I don’t like my housing?
Your housing is arranged before your come to the U.S. If, for some reason,
you do not like your housing, you are free to find housing on your own
at your own expense. Remember that your housing must be close to your
workplace, or you must have access to transportation to and from your
workplace. If you do move, you must notify Apex USA Inc. of your new address
immediately.
What is the I-20?
This computer-generated document designates your status (J-1), and is
your entrance “ticket” into the U.S. It is used for entry and re-entry
to the U.S. It is also used to record work authorizations, practical training,
and transfer approval. Keep all copies of this important document in your
passport and do not surrender it when leaving the U.S.
What is SEVIS?
The Student and Exchange Visitor Information System (SEVIS) is a government,
computerized system that maintains and manages data about foreign students
and exchange visitors during their stay in the United States. It is part
of the Student and Exchange Visitor Program (SEVP).
